Riseley Physiotherapy Pty Ltd recognises the value of sport and physical activity in building the health of a community. To facilitate such activities amongst the same community that provides Riseley Physiotherapy with its custom, the Company is making available six monetary grants of $1,000 per year. The program is a rolling program and applications can be submitted at any time throughout the year. Two rounds of Grant provision will occur each year. Outcomes will be provided as follows:

2001-2009 Selection Criteria The Riseley Physiotherapy Activity Grants’ aim is to improve opportunities for the people of the local community to participate in sport and physical activity by providing financial assistance for projects that increase or maintain the number of participants; improve the quality or safety of participation; and/or improve the range of access to participation opportunities. Applications will be considered from groups and individuals. Preference will be given to applications that benefit people who reside within the City of Melville boundaries, or within close proximity; projects that introduce and encourage physical activity to previously sedentary groups; and projects that improve the performance and skill of the physical activity Funding will not be provided for

Activity Grants Application Process All applications for the Riseley Physiotherapy Activity Grants are to occur via the Application form available below. Three hard copies of the completed
